Puppy Health and Sales Contract
The Seller, Havakiss Havanese, Donna Bianchi, and the Buyer,

1. The Seller hereby sells to the Buyer, for the amount of $____________, payment of which is hereby acknowledged, a (male/female)___________ puppy, born of
the dam ____________________________________________
and of sire ____________________________________________
All funds must clear 1 week before pick up of the puppy.
____ A. Limited Registration, never to be used for breeding purposes and to be spayed or neutered at the appropriate age.
____ B. Full AKC Registration, The puppy seems to be of breeding/show quality at the time of purchase. The breeder does not guarantee that the puppy purchased will be successful in breeding/show program. Full AKC registration gives the buyer the right to breed/show the puppy purchased when it becomes of breeding/show age. The Seller highly recommends that when the puppy is breeding age, it be examined by a Veterinarian, to be determined if the dog is of good enough health and quality to whelp or sire puppies.
1. A $200.00 deposit is due if the puppy is less than 8 weeks old and the Buyer would like to hold till the time of purchase. The deposit is NON-REFUNDABLE if the Buyer chooses not to purchase the puppy. The deposit will be deducted from the final price of the puppy
2. The Seller will register the puppy with AKC and will provide the application papers to the Buyer.
3. The Seller guarantees that the puppy is in good health on the date of purchase. The Buyer has 72 hours to have the puppy checked by a Veterinarian of their choice. If your Veterinarian finds the puppy not healthy, you may return the puppy along with the Veterinarian statement for a full refund or a replacement puppy. Please note that the Seller is not responsible for any Veterinarian/shipping costs. The Seller guarantees the puppy for the term of 1 year against life threatening genetic defects and will replace the puppy after receiving a full Veterinarian report stating the exact defect. In the event of the puppies' death a necropsy must be performed to determine the cause of death. This guarantee does not cover hypoglycemia, a condition that can be caused by stress, missing meals, excessive playing or exercise. Buyer has been fully informed about this condition, its causes and treatments.
4. The Buyer shall maintain the puppy in good condition, including adherence to a regular immunization and worming schedule, heart worm preventative, good diet, and clean and safe environment.
5. If at any time in it's life, the puppy is to be sold or given away, the Buyer shall give the Seller first option to buy the puppy back at a cost to be determined by both parties.
